A deductible is the amount you pay each year before your health plan begins to pay.
Co-pay is a small, flat fee you pay for covered care at the time of service. For example, paying $25 for an office visit. Some plans do not require a co-pay.
Coinsurance is the percent of the cost you pay for covered services. For example, you pay 20% for a doctor’s office visit or hospital stay and your plan pays 80%. Not all plans include coinsurance.
To get the most coverage, you receive care from providers and pharmacies in your health plan network. When you see provider or pharmacy out of your network, you will pay more for your care. Out of network costs do not count toward your deductible.
Out of pocket maximum is the most you would pay, each year for covered services in your network.
